How My PowerPlant collects and protects your information
Last updated: (to be confirmed by the practice)
Working draft — for practice review
This policy should be reviewed against how the practice actually operates before it is treated as final. Record retention, the patient portal and fax channels, and how we reach you by email and phone have been confirmed by the practice. Still to confirm: the last-updated date, the list of service providers (scheduling, portal, payments, labs, pharmacies), and whether any analytics or marketing tools are added later.
Who we are
My PowerPlant is the medical practice of Dr. Natashia Jones, DO, at 2260 Pool Road, Suite 100, Grapevine, TX 76051. We are responsible for the personal information we collect across the practice — in the clinic, by phone, fax, and email, through our scheduling and patient portal, through memberships and payments, and on this website. Questions about this policy can go to info@mypowerplant.org or to the phone number at the bottom of this page.
What we collect
When you become or are a patient
- Identity and contact details — name, date of birth, address, phone, email, and emergency contact.
- Intake and consent forms, medical history, medications, allergies, and family history.
- Visit notes, exam findings, treatment plans, and records of therapies provided in the clinic (for example IV, injection, and regenerative therapies).
- Lab orders and results, imaging, and records sent to us by other providers.
- Prescriptions and communications with compounding and retail pharmacies.
- Photographs taken for your clinical record, only with your consent.
- Payment and billing details, membership status, and — if you use it — insurance information for superbills.
- Messages and call notes from your contact with our team.
Your health information as a patient is also protected by our Notice of Privacy Practices, which takes priority over this policy where the two differ.
When you contact us or use the website
We collect what you give us by phone, email, fax, or through our forms — the wellness quiz, booking request, membership waitlist, and contact messages. That can include your name, email, phone number, the quiz answers you choose to give (health goals, symptoms, lab history), anything you volunteer in a message, and your consent choice with the date and time of submission.
Information collected automatically
To deliver and secure the website, our hosting platform records basic technical details about requests — such as browser type, device, pages requested, and approximate location derived from the network address. We use this to keep the site running, diagnose problems, and detect abuse. We do not build advertising profiles from it.
Information from other sources
Our scheduling and patient-portal provider collects appointment details on our behalf. Labs, pharmacies, referring physicians, and payment processors may send us information related to your care or your account.
How we use it
- To answer your question or respond to your request.
- To prepare for a possible visit and, if you consent, to send you the summary your quiz answers point toward.
- To schedule appointments and send reminders.
- To communicate with you about your care, our services, and membership availability.
- To keep the site secure and prevent spam, bot submissions, and abuse.
- To improve our services and how the practice runs.
- To meet legal, regulatory, accounting, and record-keeping obligations.
How long we keep it
Inquiries, website form submissions, and waitlist entries from people who do not become patients are kept only as long as needed to respond, follow up, and meet our obligations, and are then deleted or de-identified.
Patient medical records are kept for a minimum of seven years after your last visit — the period Texas rules require — and longer for minors. The same period applies whether the record is held in our office files or in the patient portal. Billing and payment records are kept as long as tax and accounting rules require.
How we protect it
Access to your information is limited to members of our team who need it for their work, and everyone on the team is trained on patient privacy. In the clinic, paper records and devices are kept secured and out of public view. Our systems use authenticated access, encrypted connections, role-based permissions, and bot and abuse protection on our website forms. We verify identity before discussing care or releasing records by phone.
Ordinary email and website forms are not the right place for detailed medical records, imaging, or full lab reports — please use our patient portal or fax for documents, or ask us for the best channel. If you ask us to communicate by unencrypted email, we will do so at your request after explaining the risk.
Your choices and rights
- Access, correct, or delete. Ask us what we hold about you, ask us to fix it, or ask us to delete it. We cannot delete information that forms part of your medical record or that we are required by law to retain, and we will tell you when that applies.
- Email choices. Follow the unsubscribe instructions in any message we send, or tell us directly. We may still contact you about your care, appointments, and required notices.
- No sale or sharing. We do not sell or share personal information for cross-context behavioral advertising, so there is nothing for you to opt out of.
- Do Not Track. Browsers that send a Do Not Track signal are not tracked by us in the first place, so there is nothing for that signal to change.
Children
Our website is not directed at children, and we do not knowingly collect information online from anyone under 13. When we care for a minor, information is collected from and shared with a parent or legal guardian as Texas law allows. If you believe a child has given us information, tell us and we will remove it.
Changes to this policy
We will post any change here and update the date at the top of the page. If a change materially affects how we use your information, we will say so more prominently — on the site, or by email if we have your address.
